Pre-Payment Forms
Pre-payment: The easiest way to keep you school milk program simple and to keep students drinking milk!
Adopting a pre-payment system at your school will ensure the success of the program. The form indicates options for parents to order milk for their child in easy pre-paid amounts so the worry about their kids losing money at school is no longer a concern.
Milk Punch Cards
To be used in conjunction with the pre-payment system, download and photocopy this milk punch card. Punch cards are used to collect payment for milk and to distribute the student incentives. Each time the student receives milk, the milk monitor hole-punches the card and when the card is full, the student can receive a milk reminder prize!
Parent Letter
Use this letter template to inform parents about your school's milk program, gauge interest in participation, and to to recruit volunteers. You can use the letter as a template; photocopy it and send it home with each student at your school.
